ATHLETIC FORMS 2025-2026

NJ Dept. of Education Pre-Participation Physical Exam form must be submitted to the Athletic Office. This form must be completed by the physician within the last 365 days from the start of your child participating.    Any “YES” questions on the health history page of the exam must be fully explained and a vision screen is required on the exam form or a clearance letter must be provided from an eye doctor.

The NJSIAA Physical Form must be completed every calendar year by a NJ Physician, Physician’s Assistant or Nurse Practitioner. Please only submit the last page of the form, THE MEDICAL ELIGIBILITY CLEARANCE FORM to the Athletic Department. (All NEW STUDENTS ie. ALL FRESHMAN, and students transferring from other districts must submit the entire form)

 

Forms may be dropped off at the HS main office. Please note that the Cardiac Assessment Module on the Medical Eligibility form must be signed by the medical professional who completed the physical examination. The school physician is then required to review this form before any participation by the athlete.

NJ Dept. of Education NEW Updated Health History form 

The HEALTH HISTORY UPDATE FORM is required each season/every 90 days. This form must be completed by parents/guardians  and submitted to the Athletic Department prior to the start of each season.

PLEASE NOTE PHYSICAL CLEARANCE MAY TAKE UP TO 1 WEEK FROM RECEIPT OF THE FORMS AT WH, so be sure to leave enough time for clearance. Parents/Guardians will receive a letter by email once the school physician has cleared the yearly physical examination form. Login to Genesis each season to see what is needed for clearance and to check the clearance status each season.

Private Transportation Application - to be completed and return to the Athletic Director jonesm@warrenhills.org at least 24 hours in advance of the request.  The Athletic Director will confirm if the request is granted.
